Come in for your first visit and begin building an ongoing primary care relationship.
Be Prepared
A valid government-issued photo ID.
Bring your current insurance card if you plan to use insurance.
A current list of medications, supplements, and dosages.
Be prepared to discuss your medical history, prior surgeries, allergies, and important health concerns.
Names of specialists or other healthcare professionals involved in your care, when relevant.
Prior medical records, laboratory results, imaging, or vaccination records can be helpful when available.
You do not need to arrive with every previous medical record in order to request an appointment. Our office can help guide you on what information may be useful for your visit.
